Rosebrooke Senior Living in Adams is the only CBRF in the area that is owner and operated by an Adult-Geriatric Nurse Practitioner. It is our mission to provide our residents and families with person centered care in a home like environment. Our staff is dedicated to providing quality care while showing compassion and kindness, encouraging personal choice, preserving dignity, and advocating for the residents of Rosebrooke Senior Living.

Our work environment includes:

• Private rooms with priviate bathroom and shower.
• Friendship Suites / double occupancy

Duties:
- Provide direct patient care, including administering medications, monitoring vital signs, and assisting with activities of daily living.
- Collaborate with the healthcare team to develop and implement patient care plans.
- Document patient information, including medical history, symptoms, and treatments.
- Educate patients and their families on medications, and self-care techniques.
- Maintain accurate and up-to-date patient records using electronic health record systems.
- Follow infection control protocols to prevent the spread of diseases.
- Assist with specialized procedures such as wound care.
- Provide critical care support to patients in emergency situations.

Qualifications:
- Valid LPN license in the state of Wisconsin.
- Minimum 2 years of experience as a Licensed Practical Nurse.
- Experience in critical care or dementia care is preferred.
- Proficiency in medication administration.
- Knowledge of infection control protocols and practices.
- Ability to work effectively in a fast-paced environment with a diverse patient population.
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
- Flexibility to adapt to changing patient needs and priorities.

Note: Experience working in an assisted living facility is a plus.
